Expanding My Table to Add Others

We need to ensure that people on one table are from a household or a bubble. To comply with current social distancing laws, we  are unable to mix households or bubbles on a single table.  Please fill in this form if you would like to add someone from your household or bubble to your table.  You can either add 1 or two people. We will get back in touch with you when we are in the office.
